RELEASE FORM:
I understand there is supplement preparation for this retreat. Please realize these are required for increasing the electro-vibrational frequency in the body.
I understand that I am to be alcohol and recreational drug free for at least one month before our retreat.
I understand that I am solely responsible for my health and safety, and will not hold retreat leaders or Faith in Wellness Retreat Center responsible for any loss, injuries, or illness that may occur. I will consider the interests of the group. I will adhere to the guidelines of the retreat. Please note: To insure a secure and sacred space, we do not allow alcohol or tobacco at the retreat. Thank you for understanding and agreeing.
I understand I am to attend one follow up Atom and Eve meeting after my retreat. This can either be in person or by phone/video web.
I understand and consent to photos taken during the Atom and Eve event and voluntary testimonial videos that contain my images, being used on the Faith in Wellness Facebook page or the Faith in Wellness website.
CANCELLATION POLICY: Faith in Wellness is committed to the success of our retreat participants. We host our retreats in small group settings to ensure the success of each individual in the group. Please understand that when a retreat participant cancels without giving enough notice, they may prevent another participant from being able to participate.
For this reason, if you cancel prior to 15 days before the retreat, we will give you a full refund minus 10% of the total price of your retreat.
If you cancel within 15 days of your retreat, you we will not be issued a refund, however, we will reschedule your retreat to a retreat offered within one year of your cancellation. The retreat is to be the same value or you will be expected to pay the difference in price. There is also a $75.00 rescheduling fee to reschedule your retreat.
